Termination of PBF Fellowship
Transfer to different institution
Change in Mentor
Significant change in proposed studies
A transfer or change in Mentor requires the following actions:
1. Write a letter to the PBF Fellowship Program Director and …
Explain the reason for change and the date of transfer
Provide name of new Mentor
State current Mentor will continue as a secondary Mentor
Detail how PBF Fellowship research studies will continue
Have current Mentor co-sign letter
2. Submit a letter from the new Mentor indicating the guidance and support which will be provided
3. Submit the new mentor’s NIH biosketch
4. Complete REQUEST FOR CHANGE IN STATUS FORM
5. Provide a letter from new department chair if change in institution is requested
A significant change in proposed studies requires the following actions: A letter from the Fellow and Mentor explaining the proposed changes to the studies, as well as any proposed changes to the faculty development plan and/or environment (i.e. additional space, equipment, technology, mentors, consultants or collaborators) needed to assure success.
